A mental health crisis changes how an individual functions at home, school, work, and many other aspects of the community. For those transitioning from an acute psychiatric episode –which requires hospitalization or puts them at risk for hospitalization– community stabilization services are key to long-term recovery and function. To support people in resuming life in their communities and out of the acute care setting, EPIC Health Partners deploys a two-pronged approach. In this blog, we explain this process.
1.) EPIC Health Partners Provides Mobile Crisis Response
For individuals experiencing a moderate to severe increase in symptoms of a mental health disorder, such as schizophrenia or bipolar disorder, we provide intensive recovery services. Caring, crisis-trained professionals are assigned to these patients and will deliver compassionate, patient-focused interventions and ongoing care.
Our team visits and works with adults, teens, and children at home, work, or wherever convenient for up to eight hours within a 72-hour period. They help to diffuse severe mental health symptoms –such as violent urges – with real, evidence-based solutions proven to be effective in stabilization.
EPIC Health Partners meets needy individuals where they are, providing the psychiatric assessment, access to the right medications, and the skill-building needed to keep people become better adjusted to their daily circumstances and environments. This strategy helps patients assimilate and function in life rather than end up in prolonged inpatient treatment at a psychiatric facility.
2.) EPIC Health Partners Provides Community Stabilization
This transitional community care method can run anywhere from one to seven days post-hospitalization. Community stabilization and mobile crisis response are both Medicaid-eligible in the state of Virginia. Community stabilization interventions begin in the psychiatric care hospital for people who are recovering from an acute episode of mental illness. Our trained staff provides the ways and means to phase out from the acute care setting –in a hospital or other long-term health care facility– and to successfully transition back to home or to a structured-care setting.
